Van Alstyne ISD families are asked to log in to Skyward Family Access and complete the form titled “Notice of/Consent for School Health Related Services” as soon as possible.

The form provides a detailed overview of the health-related services available at school and allows parents and guardians to indicate which services they consent for their child to receive while on campus. It expands upon the health information collected during registration, and responses will be provided directly to VAISD school nurses.

With the school year now in full swing, completing the form promptly will help prevent delays in providing health-related services to students when needed.

Families who have already completed the form do not need to take any further action.
