Effective with the 2014-2015 school year, the USDA National School Lunch Program requires each meal to include fruits, vegetables, whole grains, protein, and low-fat milk. A student has to choose a minimum of 3 components, or choose all 5, and (1) of the components must be a fruit or vegetable. The Breakfast Program requires breakfast to consist of more grains and that 4 items from the required components are offered and a student must choose 3.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation.  The completed AD-3027 form or letter must be submitted to USDA by: 1.
